That is spalling, caused by water absorbed into the brick expanding as it freezes. Individual spalled bricks can be replaced and the stack waterproofed to stop it spreading. If enough of the face is gone, a partial rebuild of the upper courses is the honest answer, and we will say so.
Leaning, a visible gap between the chimney and the house, cracked or missing liner tiles, or crumbling mortar you can pull out by hand are all reasons to stop using it until it is inspected. A camera inspection tells you what is happening inside, which is where the risk usually is.
